Job Summary Job Title: Prescribing Nurse Practitioner Location: Westwood Road Surgery, Reading Salary: Up to £64,907 (FTE), depending on experience Working hours: Part time, up to 25 hours per week Job type: Permanent Job Responsibilities Undertake triage and clinical assessment of patients, including same‑day and routine patients. Independently manage a wide range of conditions, providing evidence‑based treatment and prescribing in line with local guidelines. Conduct home visits, providing high‑quality care outside the practice setting which may include frailty checks, wound care, and medication reviews. Deliver QOF‑aligned chronic disease management. Provide medication reviews, reconciliation, and support safe medicines optimisation. Offer lifestyle advice and signpost patients to community or social prescribing services. Work closely with GPs, nurses, pharmacists, and the wider multidisciplinary team. Attend clinical meetings, safeguarding reviews, and case discussions. Provide clear, compassionate communication to patients and carers, including handling complex or sensitive information. Adhere to confidentiality, safeguarding, infection control, and information governance standards. Participate in quality improvement and audits where appropriate. Maintain accurate clinical records and complete all required documentation. Qualifications Registered Nurse (NMC) Independent Prescribing Qualification (V300) Experience working in primary care Strong IT and digital clinical system skills Understanding of safeguarding, confidentiality, and information governance Ability to work autonomously within scope of practice Experience undertaking home visits (desirable) Other Information Disclosure and Barring Service Check: This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and will require a submission to the Disclosure and Barring Service to check for any previous criminal convictions.
