Royal Primary Care Chesterfield We have an opportunity for 1 x part‑time (22.5 hours per week) Care Coordinator based at Royal Primary Care East sites, located in Chesterfield. An opportunity has arisen to join our growing team at Royal Primary Care and we are looking for an enthusiastic part‑time Care Coordinator to work within our busy medical practice in the Chesterfield area focusing on supporting patients who are in respite care at a local Care Home. Key Responsibilities Manage registrations and prescription management of a busy respite care home. Provide professional and holistic administration support for all patients discharged to the home, ensuring a smooth and safe transition from hospital to respite to home. Achieve excellent patient care via telephone and face‑to‑face interactions. Work within the Administration team with a focus on achieving excellent patient care. Please see the attached job description for a full breakdown of the duties and responsibilities. Qualifications & Skills Excellent IT skills. Good prioritisation skills. Understanding of General Practice processes or relevant transferable skills. Ability to work as part of a team. Good level of qualification (or equivalent experience). Competency in Maths and English.
