Overview This is an exciting opportunity for an experienced Learning Disability Nurse to take on a leadership role within the Complex Physical Health nursing team, as part of the Community Learning Disability Team in Greenwich. The post holder will be part of an experienced nursing team comprising of nurses and specialist support staff who work towards improving the physical health and wellbeing of adults with learning disabilities within the local borough. Responsibilities To provide visible, high calibre nursing leadership and support around the ‘Complex Physical Health’ (CPH) of Learning Disability within a multi-disciplinary framework. Lead the development of complex physical health of Learning Disability practice, promoting the clinical and professional development of staff within the community team, to ensure they provide holistic and evidence-based care. Provide specialist assessment, intervention and management, working collaboratively with colleagues, service users, families and other agencies. Lead on providing robust health liaison to acute and primary care to ensure our service users are able to access mainstream health services. To contribute to the continuing development of the ALD Service Physical Health Group. Organisation Oxleas offers a wide range of NHS healthcare services to people in community and secure environment settings. Our services include district nursing, speech and language therapy, care for people with learning disabilities and mental health care such as psychiatry, nursing and therapies. Our multidisciplinary teams look after people of all ages and we work in close partnership with other parts of the NHS, local councils and the voluntary sector. We have over 125 sites across South England. In London, we operate within Bexley, Bromley, Greenwich and Kent. Can assess and manage complex clinical issues in relation to service users; can advise and collaborate with carers, professionals, other agencies around complex clinical interventions and outcomes. Assesses, implements and critically evaluates intervention outcomes to maintain and improve the physical well-being of service users. Involves other agencies/services as appropriate to meet need. Liaises with multi-disciplinary colleagues and other professionals / agencies to address issues that create barriers to social well-being and reports on unmet needs and obstacles to senior colleagues. Develops training for staff, users and carers around health promotion. Facilitate service users and carers in overcoming barriers to accessing primary care and other NHS services.
