Job Summary We are looking for a pharmacy technician to join our team at Wyvern Health PCN (Priory Road Medical Centre, The Lawn Medical Centre and Old Town Surgery) with a passion for primary care and working with our patients and staff. You will join a PCN pharmacy team of pharmacists, pharmacy technicians and prescription coordinators who will provide support. About Us Wyvern Health Primary Care Network incorporates The Lawn Medical Centre, Old Town Surgery and Priory Road Medical Centre. The PCN is based in Swindon supporting over 26,000 patients. We are high performing practices with a strong ethos of continuity of patient care, teamwork and peer support. We have a list size of circa 26,000 patients, we use SystmOne and our most recent CQC rating was GOOD. Responsibilities Daily prescription queries – Providing support to Prescription Coordinators and Clinical Pharmacists in managing daily patient prescription requirements, including resolution of patient / pharmacy queries, out of stock medications, pharmacy queries and ETP queries. Discharge summaries – Managing and processing discharge summaries and clinical letters, undertaking patient facing and patient supporting roles to ensure effective medicines use through shared decision‑making conversations with patients. Reviewing patient medication – Running blood pressure and medicine monitoring clinics (patient facing or virtually, including in care homes), carrying out medicines optimisation tasks such as checking inhaler technique, supporting medication reviews and medicines reconciliation, and providing specialist expertise to address public health and social care needs. Vaccination programmes – Supporting vaccination programmes for patients and staff as required. MHRA alerts – Managing MHRA alerts and actions, leading receipt and understanding of alerts, communicating with PCN contacts and completing all associated actions, including audits, patient searches, patient letters. Eclipse alerts – Managing Eclipse alerts, running searches and deciding on appropriate courses of action, communicating with PCN contacts and completing all associated actions, including audits, patient searches and letters. Audits – Undertaking clinical audits, supporting quality improvement measures, contributing to the Quality and Outcomes Framework and enhanced services, assisting in the delivery of medicines optimisation incentive schemes and patient safety audits. Leadership – Acting as point of contact for ICB cost switches and savings, supporting antimicrobial stewardship initiatives, providing training and support on the legal, safe and secure handling of medicines, including the implementation of EPS, developing relationships with other pharmacy technicians, pharmacists and multi‑disciplinary team members, supporting the implementation of national prescribing policies and guidance, and providing central support for shared care protocols.
