Additionally, you will provide specialist expertise, lead changes in prescribing practices, and collaborate with community pharmacy colleagues.Qualifications & TrainingCompletion of an undergraduate degree in Pharmacy and registration with the General Pharmaceutical Council.Minimum of 5 years post-graduate experience in pharmacy.Member of the Royal Pharmaceutical Society (RPS) and working towards RPS Faculty membership.Hold an independent prescribing qualification and a postgraduate pharmacy qualification.Qualified from an accredited training pathway for clinical pharmacists in primary care settings.Behavioral Competencies and SkillsExpert Professional Practice:Demonstrates general pharmaceutical skills and knowledge.Plans, manages, and reviews pharmaceutical care programs.Follows legal, ethical, professional, and organizational policies and codes of conduct.Involves patients in decisions about prescribed medicines.Collaborative Working Relationships:Recognizes the roles of colleagues within the organization.Demonstrates effective communication with stakeholders, including patients and other professionals.Works as a team member and refers to colleagues when necessary.Collaborates with CCG colleagues and other GP Practices for patient care consistency.Leadership:Understands the pharmacy role in governance.Contributes to the workplace vision.Supports integration of general practice with wider healthcare teams.Demonstrates leadership in person-centered medicines optimization and quality improvement.Management:Understands national priorities for the team and/or service.Utilizes resources effectively.Conforms to relevant standards of practice.Identifies and resolves risk management issues.Education, Training, and Development:Demonstrates characteristics of a role model.Understands the mentorship process.Conducts teaching and assessment effectively.Participates in the delivery of formal education programs.Research and Evaluation:Identifies gaps in the evidence base.Critically evaluates and reviews literature.Generates evidence suitable for presentation at the local level.Applies research evidence into working practice.Understands principles of research governance. #J-18808-Ljbffr
