Job Summary
As a Specialist Practice Nurse you'll provide expert specialist, professional, competent and efficient nursing care based on best practice which meets patients' health needs, involving and supporting other members of the clinical team as appropriate. All duties will be carried out in accordance with the NMC Code of Professional Conduct and with regards to the various practice protocols.

Main Duties of the Job

The duties are made up of Primary Clinical Duties, Secondary Clinical Duties and Clinical Care Administration.

Job Location: This advert states one location, however this role could be at any of our 8 Surgery Sites across York.

Working Hours: The working hours are negotiable and can be discussed.

Pay: The pay is depending on experience.

Job Responsibilities

Primary Responsibilities:
Accountable for the delivery of Specialist level, quality and evidence based clinical care in accordance to professional standards, guidelines and code of conduct (Safeguarding, Nursing and Midwifery Council, NICE, Local Authority Guidelines)

  • Independent management of the defined specialist cohort of patients in line with current local and national clinical guidelines and the GMS contract.
  • Work within Priory nurse protocols as required.
  • If Nurse Prescriber, it is the nurse's responsibility to adhere to national and local guidelines, protocols and best practice associated with prescribing elements of their role.
  • Offering Specialist Chronic Disease Management dependent upon adequate training and experience.
  • Maintain stock of drugs and equipment reporting to nurse management of equipment failure/maintenance requirements.
  • Develop and contribute to internal and external audit or clinical data collation and reporting as required.
  • Contemporaneously record all Patient Contact information on the practice medical record system.
Education
  • This role requires level 1 nursing registration with the NMC and it is the post holder's responsibility to maintain all relevant elements of their professional registration.
  • The employee will be responsible for developing and sustaining own knowledge, clinical skills and professional awareness in accordance with PREP.
  • To provide specialist advice and information to patients about their condition or provide appropriate referral if necessary.
  • To drive and encourage the participation in health education carried out in the Practice for self and junior members of the team.
  • Where appropriate, prepare results of audit and present as posters or scientific presentations at meetings and conferences.

In addition, there are secondary clinical responsibilities along with administration tasks. This list is not exhaustive and may be subject to alteration, but the role includes all duties that may reasonably fall within the job purpose.

Experience
Essential:

  • Post graduate experience working in a nursing environment in primary care or within their speciality in the community or secondary care.

Qualifications
Essential:

  • Registered General Nurse with current NMC registration
  • Post Graduate qualification in the area of speciality (e.g. respiratory, diabetes, women's health)
  • Mentorship qualification

Desirable:

  • Post graduate diploma VTS completion or degree relating to primary care.
  • Non-Medical Prescriber qualification/registration
