As a salaried GP you will provide high-quality, patient-centred care while working as part of a multidisciplinary team.You will manage a range of acute and chronic conditions, participate in health promotion and contribute to team-based care plans, with appropriate supervision.The post encourages professional growth through regular feedback, mentoring and opportunities for further training in line with early-career development.Clinical ResponsibilitiesThe GP will undertake a variety of duties governed by the GMS contract and objectives of Enhanced services including:In person/telephone consultationsVisiting patients at home/in nursing homesChecking/signing repeat prescriptionsQueries, paperwork, correspondence & clinical admin as requiredWorking closely with PCN and practice teamsMake professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workersScreen patients for disease risk factors and early signs of illnessCover all clinical management of patients in surgery as appropriate including initiating investigations, reviewing results, making referrals to secondary care or to other providersDevelop care and treatment plans in consultation with patients and in line with current Practice disease management protocols as well wider guidance such as that from NICEAppropriate follow up of patientsRecord clear and contemporaneous computerised consultation notesParticipate in clinical audits, learning event analyses and practice meetingsCompile and issue computer-generated acute and repeat prescriptionsPrescribe generically whenever appropriate in accordance with the Practice prescribing formulary, NICE guidance and Good Prescribing PracticeWork with/provide support for other members of the primary health care teamPlay a lead role in the practice achieving agreed/other key performance indicator targetsIn general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.Other Responsibilities within the OrganisationAwareness of and compliance with all relevant Practice policies/guidelines both clinical and generalA commitment to life-long learning and audit to ensure evidence-based best practiceContributing to evaluation/audit and clinical standard setting within the organisationAttending training and events organised by the Practice or other agencies, where appropriate.Contributing at Practice Clinical/Organisational meetings.Person SpecificationQualificationsA medical practitioner whose name is included in the General Practitioner Register under article 10 of the General and Specialist Medical Practice Order 2003Not subject to suspension under section 41A of the Medical Act 1983Qualified GP with full GMC registration licence to practice, on a performers listEligible to be on a performers list and not suspended from that list or from the medical registerDBS clearanceHave had an annual NHS appraisal if more than one year post CCTCurrent registration with the GMC and appropriate membership of a recognised medical defence organisationMRCGPEvidence of CPD activitiesExperienceRecent experience of working in general practice in the UKCommitment to and experience of working as part of a multi-disciplinary and skill mixed team environmentEvidence of independent working in General PracticeExperience of supporting service changeEvidence of participation in AuditExperience of working to achieve standards within the Quality and Outcome Framework (QOF)SkillsUnderstand the health and social needs of a local practice patient populationCommitment to personal and professional developmentCommitment to education and trainingExcellent verbal and written communication skillsAbility to take full and independent responsibility for clinical care of patientsMotivatedFlexibleTeam playerFamiliar with clinical IT system EMIS webFamiliarity with SNOMED / READ coding in Medical RecordsConfident with additional software, excel, powerpointFlexibilityDisclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. #J-18808-Ljbffr
