Bramley Village Health and Wellbeing Centre is currently looking to recruit a full-time Advanced Clinical / Nurse Practitioner to join our team at our Bramley site.
We are a multidisciplinary practice that includes 6 GPs, GP Registrar, 4 ANPs, Clinical Pharmacists, Practice Nurses, and Health Care Assistants.
The main duties of the role would include AM and PM clinics, seeing undifferentiated patients that are a mixture of pre-booked and on the day triaged appointments.
Key responsibilities include the application of advanced clinical skills to assess, diagnose, treat, and advise patients within face-to-face and telephone consultations ensuring the use of evidence-based practice. The ANP/ACP will also lead in the development of new services and pathways to improve the quality of care and patient outcomes. This role requires dynamic clinical leadership and active participation in practice research activities to promote a culture of learning and continuous improvement within the practice.
Main duties of the job
Clinical Responsibilities:
- Patient Care: Deliver advanced and complex clinical care, providing assessment, diagnosis, treatment, and prescribing for patients with acute, chronic, and complex conditions.
- Autonomous Practice: Work independently within the scope of practice to manage complete clinical episodes, using advanced diagnostic skills and clinical judgment.
- Case Management: Manage patient cases from initial assessment to conclusion, including follow-ups and referrals, ensuring continuity of care.
- Prescribing: Utilize prescriptive authority responsibly following current guidelines and protocols to enhance therapeutic outcomes for patients.
- Diagnosis and Treatment Planning: Utilize clinical expertise to diagnose health conditions based on face-to-face and eConsultations. Prescribe medications, suggest lifestyle changes, or recommend further diagnostic tests as appropriate.
- Referral Coordination: Refer patients to other healthcare services when more detailed investigations or specialist care is needed, ensuring a seamless transition from virtual to in-person care.
Bramley Village Health and Wellbeing Centre is one of 3 sites across Leeds that are managed by the Fuller and Forbes Healthcare Group. As well as the two other sites in Leeds, Cottingley and Middleton Park, the healthcare group currently operates a total of 17 sites in Gateshead, Burnley, and Devon providing care to 120,000 patients.
Our 3 Leeds-based surgeries provide GP services to our fast-growing population of 17,000 patients. Therefore, this role presents an exciting opportunity for any suitable individual who enjoys the demands and rewards of a fast-paced and continually expanding environment.
The organisation is patient-driven and has used patient feedback in the past to develop innovative services such as a bespoke smartphone app, patient-centred practice websites, and a bespoke e-consultation service. The e-consultation service is accessible through both the smartphone app and websites.

Summary
- To provide care to patients at the point of entry to the healthcare system, including those with undifferentiated and undiagnosed health problems.
- To provide continuing care to patients across the whole practice population.
- To provide health management services to the well, offering an educational and supportive setting for patients to explore their health needs.
- To manage urgent tasks and queries received by the wider team on a daily basis.
- To provide telephone and face-to-face clinics.
- If required, to support the e-consultation service which operates nationally (training provided).
- To work as an autonomous clinical practitioner.
- Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a patient management plan.
- Conduct comprehensive clinical assessments of patients using eConsultation tools. Evaluate patient symptoms and medical history with any other relevant information provided through the digital platform.
- Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based national and practice protocols.
- Undertake telephone consultations for patients presenting with an undifferentiated diagnosis to include consulting, assessing, diagnosing, implementing, and evaluating treatment.
- Provide interventions such as referrals and relevant prescribing.
- Request and interpret tests appropriate to patients' needs.
- Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care.
- Assess, identify, and refer patients presenting with mental health needs in accordance with the national and local guidelines for Mental Health services.
- Utilize and demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment.
- Communicate with and support patients who are receiving difficult or complex information or results in a professionally appropriate and supportive manner.
- Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background, and preferred ways of communicating.
Additional Criteria
Essential
- Clinical Skills:
- Extensive experience in managing complete episodes of clinical care including assessment and diagnosis and prescribing.
- Advanced clinical skills in a primary care setting with proficiency in using clinical diagnostic tools and conducting complex clinical procedures.
- Communication and Interpersonal Skills
- Excellent communication skills with the ability to convey complex information to patients and team members effectively.
- Strong interpersonal skills capable of working collaboratively within a multidisciplinary team.
- IT Proficiency
- Competence in using SystmOne, essential for effective record-keeping and data management.
- Research and Development
- Involvement in clinical research or audits with the ability to apply findings to improve clinical practice.
- Skills in quality improvement methodologies applicable to primary healthcare.
- Specialist Knowledge
- Further qualifications in a specialist area such as diabetes or dermatology or emergency care.
- Experience and competency in minor surgery procedures or chronic disease management.
Essential
- Nursing degree or other relevant nursing qualification granting professional license to practice primary care nursing.
- NMC/HCPC/GPhC professional registration.
- Masters degree or postgraduate diploma in advanced practice.
- Remote consultation qualification.
- PDP development demonstrating remote consultation and/or personal quality improvement.
Essential
- Must have completed independent prescriber qualification.
- Confidently use a laptop to provide care.
- Experienced in providing telephone consultations.
- Have at least 2 years experience providing remote care to patients as a clinical practitioner.
- Experience of SystmOne.
- Experience of EmisWeb.
- Experience of providing video consultations.
