Job SummaryThe ideal candidate would be a qualified Practice Nurse with some Long Term and Chronic Disease Management experience and ideally specialising in COPD/Asthma (but not essential as training can be provided).Main duties of the jobManage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients.Identify, manage and support patients at risk of developing long‑term conditions.Provide advanced, specialist nursing care to patients as required in accordance with clinical evidence, NICE and the NHS SBT.Provide wound care (ulcer/Doppler, etc.).Provide specialist clinics such as contraception.Process and interpret pathology and other test results.Provide chronic disease clinics.Maintain accurate clinical records and chronic disease registers.Implement immunisation programmes for adults and children.Support the team in dealing with clinical emergencies.Contribute to practice targets (QOF, etc.).Support the clinical team with safeguarding matters, in accordance with local and national policies.Deliver opportunistic health promotion where appropriate.About UsWoodbrook Medical Centre is a thriving market‑town GP surgery in Loughborough with a multi‑disciplinary on‑site team, a friendly informal atmosphere, competitive salary and excellent educational opportunities. The practice is well‑managed, has a good CQC rating, and is a research practice serving a diverse population across Leicestershire, Nottinghamshire and Derbyshire.Job ResponsibilitiesAs an autonomous practitioner, you will deliver practice nursing services within a multidisciplinary team, focusing on health promotion, chronic disease management, well‑woman/well‑man clinics, and supporting the management team in reviewing clinical policies and procedures.Essential QualificationsRegistered Nurse.Qualified Triage Nurse.Experience of chronic disease management.Diploma in Nursing.Asthma NQF Level 5 or equivalent (or working towards this qualification).COPD NQF Level 5 or equivalent (or working towards this qualification).Desirable QualificationsExperience working in a primary care environment.Experience as a practice nurse or community nurse.Wound care expertise.Feno Training.Eden Diabetes qualifications.Knowledge and Skills – EssentialExcellent written and oral communication skills.Strong IT skills.Accurate note‑taking.Using Arden templates on SystmOne.Clear and polite telephone manner.Effective time management (planning and organising).Ability to work as a team member and autonomously.Good interpersonal skills.Knowledge and Skills – DesirableCompetent with Microsoft, SystmOne, Ardens and AccuRx (training will be provided).Experience in audit and ability to lead audit programmes.Experience with clinical risk management.Knowledge of public health issues in the local area.Awareness of health promotion strategies.ExperienceExperience in chronic disease management especially in asthma and COPD.Experience managing diabetes.Disclosures and VerificationApplicants must have current UK professional registration and are required to undergo a Disclosure and Barring Service check. #J-18808-Ljbffr
