The post holder will be responsible for a defined caseload of children and young people with additional and complex healthcare needs, ensuring their health requirements are safely identified, assessed, planned, met, and evaluated in collaboration with the multi-disciplinary team across community and educational settings.2. Main Duties and ResponsibilitiesClinical Practice:To manage a defined caseload, providing specialist nursing advice and support to children, young people (aged 3-11, potentially up to 19 years), their families, and carers.To undertake comprehensive assessments of childrens health needs, using evidence-based knowledge to formulate, implement, and evaluate specialised programmes of care.To provide skilled, hands-on nursing care, including but not limited to the management of enteral feeding devices, administration of buccal midazolam, and other complex clinical procedures as required.To act as the named nurse, taking responsibility for the planning, delivery, and evaluation of care for each child on the caseload.Training and Development:To design, deliver, and evaluate clinical training for non-health staff (e.g., school teachers and support assistants) to ensure the safe management of childrens health needs within the school environment.To act as a specialist resource and provide ongoing support and guidance to education staff and other professionals.Communication:To communicate complex and sensitive information effectively and compassionately to children, families, and carers, ensuring they are fully informed and involved in care planning.To liaise and collaborate effectively with all members of the multi-disciplinary team (MDT), including GPs, therapists, social workers, and other health professionals, to ensure integrated, child-centred care.Professional Responsibilities:To adhere to the NMC Code of Conduct and all trust policies and procedures.To maintain accurate, contemporaneous, and comprehensive patient records.To participate in clinical supervision and engage in continuous professional development.To actively participate in safeguarding procedures, acting as an advocate for vulnerable children and following local safeguarding policies.3. To communicate effectively with children, their families / carers and the wider multi-disciplinary team.Responsible for children on the identified caseload and will assess, manage, plan and evaluate care as appropriate.
