What we do is not easy but it’s very worthwhile”Clinical ColleagueResponsibilities:Provide a high level of clinical expertise, to support and facilitate individualised patient centred care to patients with life limiting illnessProvide care and support for relatives and carers in collaboration with the MDT.Facilitate and support the implementation of evidence-based practice and contribute to the ongoing development of the service.Participate in a seven day per week operational service.Demonstrate leadership through clinical expertise and specialist knowledge, delivering high standards of personalised care.Identify needs through a holistic assessment and co-creating a care and support plan with people living with cancer and other life limiting illnesses.Independently manage own caseload of patients and associated responsibilities.Independently manage the role of duty nurse on rotation with colleaguesAssist with the smooth running of the Community Team as a team lead in consultation with the Community Services Manager.Minimum Requirements:UK Registered NurseMinimum 4 years relevant post-registration experience.Experience in the supervision of junior staff/othersThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.We are passionate about our colleagues’ careers.
