We are looking for a Practice Nurse for 20 hours per week on a fixed term two year contract. Main duties of the job The post holder will work collaboratively with the multi-disciplinary team to meet the needs of the practice and patients, and will play an integral part in delivering the KPIs for the Practice. You will have a varied caseload, including: Chronic Disease Management, preventative care, immunisations, patient education. The post holder will be responsible for leading on the practice Infection Prevention and Control audits. Job responsibilities The Practice Nurse will work collaboratively with the General Practice team to provide and develop high quality, evidence based nursing services to the practice population. This role involves managing long-term conditions, delivering health promotion, disease prevention and supporting patients in self-management. The post holder will play a vital role in delivering holistic patient care, aligned with the NHS values of compassion, respect and commitment to quality of care, while working in a friendly, supportive and forward thinking team environment. Key Responsibilities Clinical Care Deliver General Practice Nursing services including chronic disease management (e.g. Diabetes, Asthma, COPD, Hypertension) Undertake cervical cytology, immunisations and vaccinations (child and adult), ECGs and health screening Provide health education and lifestyle advice to promote well-being and disease prevention Support patients in understanding and managing their long-term conditions through evidence based care and self-management plans Participate in infection prevention and control, vaccine storage and cold chain maintenance Perform clinical audits, maintain accurate records and ensure compliance with CQC, Infection Control and Safeguarding standards Participate in Quality and Outcomes Framework (QOF) and Enhanced Services Delivery Leadership and Teamwork Work collaboratively with GPs, HCAs and the wider multidisciplinary team Support the development of nursing services within the practice and contribute to team meetings Supervise and mentor junior staff (e.g. phlebotomist, GPA, HCA) when appropriate Contribute to a positive, respectful and inclusive workplace culture Professional Development Maintain clinical competence through ongoing professional education, CPD, and NMC revalidation Participate in appraisal and reflective practice Engage with clinical supervision, in-house training and external learning opportunities Take responsibility for personal learning and performance, working towards agreed objectives Governance and Quality Work within the policies, protocols and clinical governance framework of the practice Ensure safe, effective and efficient clinical practice consistent with national and local guidelines Support quality improvement initiatives and practice development projects Maintain confidentiality and data protection in line with GDPR and NHS standards Person Specification Experience Minimum 1-2 years post-registration nursing experience, Previous experience of working as a Practice Nurse in General Practice, Experience of Chronic disease management, Immunisation and vaccination experience, Cervical Cytology training and updates. Experience of leading on Infection Prevention and Control Audits. Skills and Knowledge Excellent clinical assessment and decision making skills, Ability to prioritise and manage workload effectively, Excellent communication and interpersonal skills. Knowledge of QOF, Clinical Audits, and CQC requirements, Experience using EMIS Web or similar clinical systems, Health promotion and community engagement experience, Mentorship / Supervision of junior staff or students. Qualifications Registered Nurse (RGN) with current NMC registration, Evidence of continuing professional development. Diploma or degree in Practice Nursing or equivalent, Independent Nurse Prescriber or working towards.
