About The Role In this rewarding community‑based position, you’ll provide high‑quality nursing care, advice and support to house‑bound patients in their own homes and care homes—helping them maintain independence, enhance well‑being, and avoid unnecessary hospital admissions. Key Responsibilities Deliver expert clinical care to patients in their own homes Provide strong leadership and day‑to‑day management to a small team of Community Staff Nurses and Healthcare Assistants Act as the clinical lead for the caseload, ensuring all patients receive appropriate assessments and personalised treatment plans Work independently with minimal supervision while collaborating closely with the wider multidisciplinary team Uphold all clinical policies, procedures and best‑practice guidelines Why Join Us?
