Practice Nurse Gratton Surgery, a multidisciplinary general practice serving 7,200 patients, is seeking an experienced and enthusiastic Practice Nurse. Job Summary Responsible for delivering safe, effective, evidence‑based primary care. Services include patient assessment, chronic disease management, health promotion, injury management, and infection control, with an emphasis on autonomous practice within the NHS framework. Key Responsibilities Provide direct nursing care: assessment, treatment, wound care, ECGs, blood tests. Run health screening and promotion clinics. Manage chronic disease pathways (e.g., respiratory, diabetes, hypertension, cardiovascular, cervical cytology, vaccinations, learning disabilities, severe mental illness). Lead specialist clinics such as Asthma/COPD, Diabetes, Hypertension, Cardiovascular disease, Cervical cytology, Travel vaccinations, Learning disabilities, SMI. Maintain accurate clinical records in EMIS/Docman, support data collection and audit. Support infection control, audit and quality improvement activities. Respond to emergency situations: anaphylaxis, resuscitation. Coordinate with multidisciplinary team, external agencies, and patient liaison. Engage in continuous professional development and NMC revalidation. Participate in team meetings, practice development and policy review. Essential Qualifications Registered Nurse with current NMC registration. Competence in chronic disease management. Up‑to‑date emergency (anaphylaxis, CPR) training. Knowledge of infection control, health promotion. Proficient with EMIS/Docman and general IT. Strong communication, teamwork, and organisational skills. Willingness to learn new skills. Desirable Qualifications Experience in primary care or community nursing. Certificates in cervical cytology, immunisation, travel health. Experience with EMIS / SystmOne. Audit or quality improvement project involvement. Mentorship training or willingness to undertake. Personal Attributes Professional, motivated, autonomous and able to work in a multidisciplinary team. Excellent communication, organisational and interpersonal skills. Flexibility and adaptability to evolving practice needs. Commitment to high‑quality patient care and continuous improvement.
