What You Will Do Manage a varied and complex caseload while receiving guidance from the Lead Practitioner, ensuring accountability and professional practice. Conduct comprehensive needs assessments in line with the Care Act 2014, promoting independence and identifying viable support solutions. Engage with specialist safeguarding colleagues, contribute to safeguarding enquiries, and focus on the individual’s desired outcomes. Act as a deputy for the Lead Practitioner when required, attending multi‑disciplinary meetings and representing the team. Mentor and supervise both non‑registered social care practitioners and social work students, fostering a culture of support and professional development. Develop value‑for‑money care packages in collaboration with individuals and caregivers, meeting eligibility criteria and maximizing quality of life. Work closely with individuals, families, and communities to facilitate informed decisions, contributing to effective service planning. Keep abreast of available resources and services, identifying gaps in provision, and operating within delegated budget mechanisms.
