Job Title: Community Nurse (Band 5) Location: Hounslow Band: Band 5 Contract Type: Temporary Salary: £18.01 – £28.73 per hour and 0.45p per mile paid Shifts: Flexible (days, weekends) Key Responsibilities Deliver high‑quality nursing care in the community, empowering patients to maintain independence. Assess, plan, implement, and evaluate person‑centred care plans. Collaborate with multidisciplinary teams, GP surgeries, and acute trusts. Assist in facilitating smooth hospital discharges and reducing readmissions. Essential Qualifications NMC registration (valid and active). A minimum of 6 months of NHS experience as a Community Nurse. Eligibility to work in the UK.
