Community Support Worker Job Summary An exciting opportunity has arisen for a Community Support Worker to join our Neighbourhood Mental Health Team (NMHT). The NMHTs are at the forefront of providing accessible care, integrating both primary and secondary mental health services, in line with NHS long‑term objectives. There are five Neighbourhood Teams in Herefordshire. The post advertised is to work within the South & West NMHT. Each NMHT is comprised of Mental Health Practitioners, Support Workers, Administrators, Clinical Psychologists and a Consultant Psychiatrist. Additionally, the teams are supported by Mind Link Workers and Vocational Employment Specialists from Landau, with the aim of providing a holistic, recovery‑focused service. We work closely with our Primary Care Networks, demonstrating good relationships and communication with our respective GP partners. The teams are supportive, with regular MDT meetings, individual/group supervisions and team away‑days. For more information please contact the Recruiting Manager. Contract & Sponsorship This is a full‑time role. Please note the Trust will not routinely provide sponsorship to work for this role. In addition to meeting the Home Office salary threshold you must have significant experience to be offered top of the pay band and work a minimum of 35.5 hours per week. Salary & Band Band 3 – £24,937 to £26,598 per year. Responsibilities Provide mental health support in the community, implementing care plans and utilising personal autonomy and the support of the Multi‑disciplinary Team (MDT) when needed. Work with a range of agencies within the NMHT and externally, sign‑post patients to appropriate sources of support or provide short, moderate or longer‑term interventions based on clinical need. Support the running of the service, manage a caseload, attend meetings and take on additional responsibilities as and when required. Travel throughout Herefordshire to provide patient care and multi‑agency working, including support at patients’ own homes. Provide supportive treatment of working‑aged adults with mental health problems, ranging from a primary to a secondary level of need. Complete statutory and mandatory training as required and keep this updated. Work according to the Trust values, policies and procedures, and provide support to carers. Undertake extra duties as required, depending on the needs of the service. Person Specification Experience (Essential) Previous relevant experience working with the client group. One‑to‑one working. Personal care tasks. Experience (Desirable) Working in collaboration with VCSE service. Experience delivering evidence‑based psychosocial interventions. Additional Criteria (Essential) Will be required to complete breakaway / de‑escalation training. Able to meet the travel requirements of the post. Not routinely sponsored for work. Qualifications (Essential) Level 3 qualification (e.g., A‑level, NVQ3) in Care or equivalent. Level 2 qualification (e.g., GCSE, NVQ2) in Care or equivalent and will be expected to undertake the relevant Level 3 qualification on appointment to post. Expected to meet the Care Certificate standards within 12 weeks of commencing employment. Will be required to provide original certificates to verify qualifications at point of offer. Qualifications (Desirable) Medication administration qualification. Additional qualifications regarding the care of others. Knowledge (Essential) Knowledge of different types of mental health problems and presenting symptoms. Knowledge (Desirable) Knowledge of the Mental Health Act 1983, Mental Capacity Act 2006 and other relevant legislation. Skills & Abilities (Essential) Ability to challenge poor behaviour. Role modelling good behaviour. Positive attitude and ability to act on feedback. Ability to praise and support others. Clear and effective communication in English, both verbal and written. Follow a plan of care completed by a senior clinician and support the patient to achieve recovery goals. Adaptable. Recognise own limits. Use own initiative. Good team player.
