Family Hubs Health Practitioner Location: Locala Health & Wellbeing – Calderdale and Kirklees 0-19 services. 2 positions at Kirklees Gateway and 2 positions at Calderdale Family Hubs Single Point of Contact. Job Role Band 5 (indicative) – £31,049 – £37,796 37.5 hrs per week Permanent Responsibilities Act as first point of contact for children, young people and families reporting concerns in Kirklees and Calderdale. Work within a multidisciplinary team. Deliver support and guidance to families and professionals in an engaging, caring and non‑judgmental manner. Practice professional curiosity when handling calls to achieve a desired outcome, safely and effectively, and handle information in confidence. Embed a "whole family" approach in line with the early help strategy, collaborating with local authority, health, education and voluntary sector partners to reduce escalation to statutory services.
