Community Practitioner

The Community Treatment Team provides comprehensive, person‑centred support to adults experiencing complex or enduring mental health difficulties. Working as part of a dynamic, multi‑disciplinary team, the post holder will contribute to the delivery of high‑quality, recovery‑focused care within the community. The role involves working collaboratively with service users, their families, and other health and social care professionals to promote mental wellbeing, independence, and social inclusion. The post holder will undertake assessments, develop and implement care plans, monitor progress, and respond to changing needs, ensuring that all interventions are safe, effective, and aligned with evidence‑based practice. This position requires strong interpersonal skills, professional judgement, and a compassionate approach to supporting individuals to achieve their personal recovery goals. The successful candidate will also contribute to service development, risk management, and the ongoing improvement of community‑based mental health care. Responsibilities Deliver high‑quality, person‑centred mental health care in community services Undertake comprehensive risk assessments and care planning Attend Mental Health Act tribunals and professionals' meetings as required Prepare formal clinical reports for tribunals Provide support and input during mental health crises and implement robust risk management plans Mentor and support junior staff, students, and colleagues Liaise effectively with GPs, social workers, carers, and other service providers Essential Qualifications Professional Qualification and current Professional Registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist Mentor Training Evidence of post‑registration training in clinical/psychological interventions/skills relevant to the post Proven track record of post‑registration clinical experience relevant to the post Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi‑Agency Panel for the Protection of Vulnerable Adults) and Child Protection Up‑to‑date knowledge/experience of relevant legislation Awareness and understanding of professional accountability, NMC standards and other local and national initiatives 12 to 24 month band 5 qualified nurse experience dependent upon service need Desirable Qualifications Knowledge of evidence‑based treatments for psychosis and non‑psychosis Experience of working within a community mental health setting Knowledge of evidence‑based interventions such as Structured Clinical Management, CBT, Family Interventions Clinical Leadership Knowledge of other services linked to community services, including voluntary and statutory agencies Training & Induction of staff Teaching/Presentation skills Essential Skills Adaptability, flexibility, excellent communication skills and commitment to multi‑agency team work Ability to provide evidence‑based therapeutic interventions Comprehensive skills for continuous assessment of risks/safety planning skills Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs Ability to work autonomously and within a team Evidence of a high level of professional, organisational and leadership skills (self & others) Desirable Skills Experience of working within a community mental health setting Knowledge of other services linked to community services, including voluntary and statutory agencies Training & Induction of staff Teaching/Presentation skills We aim to attract and retain a diverse, talented and committed workforce, who are caring and compassionate, and therefore able to meet the demands of the modern NHS now and in the future.
