Key Responsibilities Ensure the care co‑ordination of a designated caseload of clients with severe and enduring mental health problems. Ensure that Care and Treatment Plans are adhered to and that standards are met in line with the Mental Health Measure (Wales) 2010. Act as a point of contact for GP's and other interface services, offering advice, support and signposting to appropriate agencies. Formulate and develop standard and complex outcome‑focused care and treatment plans. Ensure a comprehensive risk assessment is completed and documented for actual and potential risk. Assist with the running of the depot/clozapine clinic and administer and supervise long‑acting injections; complete physical health assessment and side‑effect monitoring for patients prescribed Clozapine. Assess individuals in crisis, complete comprehensive assessments of health and social care needs, agree the most appropriate course of action, evaluate and manage clinical risk, and formulate a plan of care and deliver evidence‑based crisis interventions as appropriate. Work collaboratively with service users, carers and partner agencies to deliver high‑quality care.
