The Community Learning Disability Service provides integrated specialist health and social care to adults with learning disabilities in Tower Hamlets. Our service also runs a specialist day activity project – CREATE. We are offering a Band 6 nursing post within our established multi‑professional team. The successful candidate will be RNLD or RGN qualified and will support people with complex health needs. Key responsibilities Use nursing skills to support people with learning disabilities and autism to maintain, promote and improve health. Assess and support people with learning disabilities, complex health needs, autism and common co‑existing conditions such as ADHD and mental illnesses. Fully involve the person with a learning disability in decisions, especially those at risk of challenging behaviour. Help people with learning disabilities develop their full potential by reducing the impact of their impairment, which could limit their social inclusion. Facilitate access to health services through collaboration and partnership with primary and secondary health services and other relevant providers. Use a person‑centred approach to promote choice, independence and inclusion. Contribute to shaping local health services so that they are organised and run in an inclusive way for people with learning disabilities. Undertake assessments, reviews and care planning for NHS Continuing Healthcare Funding in accordance with the National Framework for NHS Continuing Healthcare. Understand the implication of the legal and policy framework underpinning the support of people with learning disability and autism. Working for our organisation The post holder is line‑managed by the Care Team Manager (Integrated Health & Social Care) and is professionally accountable to the Clinical Nurse Specialist (Lead Nurse). The role requires maintaining multi‑professional working relationships within the CLDS, primary and secondary care services, and other agencies that support people with learning disabilities in Tower Hamlets. Key working relationships: Care Team Manager (Integrated Health & Social Care – line manager) Clinical Nurse Specialist (Lead Nurse – professional accountability) Primary and secondary care services and partner agencies (multi‑professional collaboration) The post falls within the Criminal Records Disclosure Policy – a criminal record check will be applied for a successful candidate.
