The Licensed Practical Nurse (LPN) in Home Care works as a member of an interprofessional health care team utilizing the nursing process to provide direct nursing care to individuals across the life span. Working as a Case Manager, the LPN is accountable for assigned caseloads with the primary responsibility to assess client needs, determine service needs, negotiate service options, care planning, and coordination of transitions across care settings. The LPN also performs interventions and restricted activities for clients as required according to the client need. There is also an expectation to be a mobile worker. This role requires travel up to 100 km/day caring for clients in their homes or in Rural community. You will be responsible to operate a laptop computer and mobile phone. Ability to utilize critical thinking skills in the practice of conducting initial and ongoing nursing assessments, care planning and service delivery. Ability to utilize critical inquiry to assess, plan and evaluate the effectiveness of interventions on client outcomes. Excellent verbal and written communication skills Basic use of technology and competency in computer, clinical electronic device use, and electronic documentation.Preferred QualificationsCompletion of a post-basic Medication Administration Course for LPNs. Recent work experience in a community-based setting, Home Care experience preferred. Experience working with complex chronic disease(s) and Palliative clients.
