Can you lead a team of nursing staff compassionately and inclusively? We are seeking someone who is adaptable and collaborative, with the potential to grow, and the ability to plan, prioritise and organise their work, to join our neighbourhood nursing service. Key Responsibilities Provide nursing care for housebound patients aged 18 and over within the Medway area, supporting complex and long‑term conditions. Assess, manage, plan and deliver care, while maintaining professional and legal accountability. Lead and manage a Neighbourhood Nursing Team in Strood, geographically aligned to a locality and responsible for caseload management. Work as part of the integrated health care team, liaising with GPs, other professionals and statutory and voluntary agencies. Operate within a 7 am – 7 pm service, covering shifts 7 am‑3 pm, 8 30 am‑4 30 pm and 11 am‑7 pm, 365 days a year. Ensure high standards of nursing care and support patients and families in the community. Qualifications and Experience Recognised degree in adult nursing. Registered Nurse on Part 1 of the Nursing Midwifery Council register. Evidence of further study. Mentorship or training qualification. District Nursing qualification / community practitioner pathway or willingness to commence within 1 year. • Nurse prescriber or willingness to undertake training. Post‑graduate qualification in elderly care, chronic disease management, long‑term conditions, or rehabilitation. Post‑graduate accumulative experience in elderly, rehabilitation, community, intermediate care or acute illness. Experience of delivering education and teaching. Proven experience in an autonomous practitioner role. Experience of working in community healthcare. Experience and competence in high‑level clinical assessment of complex patients. Specialist knowledge in minor illness management, community nursing and advanced practitioner skills. Experience of using technology to facilitate patient care. Motivational interviewing and change management experience. Knowledge of current issues and trends within community healthcare, urgent care and the NHS. Working knowledge of admission avoidance ethos and timely discharge facilitation. Clinical governance and risk management awareness. Advanced level practice skills. Experience of lone working and decision making. Mentoring students and other healthcare professionals. Caseload management skills. Intermediate IT skills (Windows, Internet, Outlook, Excel, EPEX). Presentation skills and teaching qualification or experience. Research awareness and audit skills. Use of research and evidence to inform practice.
