Toprovide cover for other pharmacists within the relevant Clinical PharmacyTeam as appropriate. To support the work of the Lead Pharmacist for the areaby assisting in the production of drug-related policies, procedures andfinancial reports, and by performing horizon scanning activities.Wehave an exciting opportunity for an motivated pharmacist to join our pharmacyteam. If you do experience a delay in theshortlisting stage of the recruitment cycle, please bear with us while thisprocess is completed, and contact the named contact if you have anyquestions.Job responsibilitiesProvide specialist clinicalpharmacy service to patients and the multidisciplinary team within eachrotation, as part of a team of pharmacists working in that speciality beingprofessionally accountable for their actions.Provide drug-related advice andinformation to patients, consultants and other health-care professionalswithin a Clinical Management Team.Monitor the clinical use of drugsin patients. Advising medical and nursing staff how to optimize drug therapy.Reporting prescribing and administration near misses, and risk managementissuesProvide education to healthcareprofessionals within the specialityParticipate in the training ofjunior pharmacy staff within the specialitySupport the Lead Pharmacist for thearea with the production and implementation of drug related policies andprocedures, with the production of financial reports and with horizonscanning activities.Participate in operational servicecommitments to dispensaries, Medicines Information and aseptic servicesThe post holder will be required towork evenings, weekends and Bank Holidays on a rotational basis andparticipate in the on-call from home pharmacists rota.Person SpecificationQualificationsMasters qualification in Pharmacy, or equivalent qualification, plus completion of pre-registration trainingPharmacist Registrant of the General Pharmaceutical Council (GPhC)Postgraduate Diploma in Clinical Pharmacy or equivalent experience or qualificationExperienceThe ability to interpret information and provide adviceModerate or significant post registration experience as a practising pharmacist in hospitalBroad clinical pharmacy knowledgeSkills & behavioursTeam playerAbility to work alone and under pressureFlexibleCommunication, written, listening and verbalAbility to prioritiseAbility to work under pressureDisclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. #J-18808-Ljbffr
