Are you a confident and motivated healthcare professional who works well in a multi-disciplinary team environment? Stevenage South PCN are seeking a skilled advanced practitioner to join our supportive team and deliver high-quality, safe, patient-centred care.Based in Stevenage, Hertfordshire, this position is offered on a part-time basis for 16-22.5 hours/ 2-3 days per week (exact days to be agreed with the right applicant).This is a contract position, fixed-term until 31st March 2026 but has the potential for extension.Main duties of the jobConduct thorough clinical assessments and formulate management plans for patients with minor illnesses.Assess, diagnose, and manage patients presenting with a variety of minor illnesses across primary and urgent care settings.Prescribe independently, providing efficient and effective treatment, reducing waiting times and improving patient outcomes.Work collaboratively with GPs, nurses, pharmacists, and other team members to deliver seamless care.Provide health promotion advice and support patient self-care.Maintain accurate clinical records and comply with all governance and regulatory standards.About usStevenage Healthcare Ltd is a new company which has been tasked with providing delivery and support for Stevenage South Primary Care Network.The company will provide development and delivery of SSPCN contracts and areas of work, as well as explore wider healthcare opportunities to support local people and provide positive health outcomes for our population.Why Join Us?Supportive and friendly multidisciplinary team environment.Access to clinical supervision and training resources.Make a real difference by providing timely care to your local community.If you are passionate about delivering excellent patient care and have the skills and qualifications to thrive in this role, we would love to hear from youFor an informal discussion, please contact Matt Charles on 07951 062692.Job responsibilitiesIfyou would like to find out more information about this role, please do nothesitate to get in contact with us.Person SpecificationQualificationsRegistered healthcare professional (Nurse, Pharmacist, Paramedic, or other allied health professional) with a valid Independent Prescribing qualification.ExperienceExperience and training in minor illness assessment and management.Strong clinical reasoning skills and sound knowledge of current prescribing guidelines (e.g., NICE).Excellent communication and interpersonal skills, with the ability to work well within a team and support fellow colleagues (clinical and non-clinical)Commitment to ongoing professional development and high-quality patient care.Disclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.£40,000 to £55,000 a yearpro-rata, dependent on experience #J-18808-Ljbffr
