Exercise Physiologist Pulmonary Rehabilitation

The Exercise Physiologist will play a key role in the Community Respiratory Team (CRS) in West and Central Sussex, providing Pulmonary Rehabilitation (PR) services to our patients, both through face-to-face classes, virtually and in the home of the patient. Our PR programmes and promotion of active lifestyles can make a significant difference to the lives of individuals with chronic lung disease, and the post holder will play a key part in triage, assessment, treatment delivery and post-program assessment. The post holder will work with each patient individually to gain an understanding of their condition and requirements and how PR and engaging in active lifestyle changes will support them to make the very best of living with a long-term pulmonary condition. Responsibilities Maintain own personal professional development, identifying educational and clinical training needs in conjunction with lead physiotherapist and/or senior members of staff. Lead on the provision of PR services both face-to-face and virtual. To undertake assessment of patients with diverse or complex physical, psychological, cognitive and behavioural conditions in order to deliver appropriate exercise therapy. Communicate with wider MDT's, service users, carers, stakeholders, the public and their representatives, ensuring effective & accurate information is delivered. To be responsible for identifying equipment faults, and directly resolve fault issues by reporting them in a timely manner to senior staff members. To be responsible for the day-to-day running of the area in which they are working. To make contact with patients via telephone to rebook appointments for DNA's each day to suit service availability and patient choice. To collect, collate, and disseminate complex and sensitive data relating to patients results to relevant health professionals and teams as required. To communicate efficiently on the telephone with all staff, patients, and visitors. To be cost-effective in the use of equipment and resources. To assist in audit & research as necessary. To engage in supervision and education of junior staff and students. To actively contribute to the clinical knowledge share programme in CRS.
