Our service supports individuals in self-contained apartments, enhanced by the latest assistive technology. We aim to support individuals to live as independently and confidently as possible. What you’ll do You will provide outcome-focused, person-centered support that promotes independence and maximises opportunities for the people we support. Your role will include: Supporting with medication management Teaching and encouraging life skills Assisting with daily living tasks Facilitating community access and engagement Providing personal care Helping manage tenancies Liaising with other healthcare professionals to ensure clients receive the best possible support Supporting clients with personal care Who weor looking for We need full-time (37.5 hours) and part-time (23.5 hours) staff ready to work across day and night shifts, working around a 7-day rota and you will be required to be on call on a rota basis. Additional Information All appointments are subject to an enhanced DBS check and satisfactory references.
