Job Summary Join to apply for the Community Support Worker/Trainee role at NHS. An exciting opportunity has arisen for a support worker/trainee to join the assertive outreach and recovery team which supports patients in the community with their mental health recovery. Main Duties The post holder will work with patients in their homes, community clinics, and in reach to patients admitted to hospital to support discharge home. Working as part of a multi‑disciplinary team, the role delivers high quality patient‑focused care in a community setting, improving lives of people with mental health issues through recovery promotion, wellbeing and some physical health interventions. Job Responsibilities The successful candidate must have completed the Diploma in Health and Social Care Level 2 or equivalent and have Literacy and Numeracy Level 2 or equivalent. Within the first six months, the candidate must also complete the Trust Healthcare Assistant competencies. Candidates lacking qualifications are required to enrol in the Healthcare Assistant Framework, which includes obtaining the Diploma in Health and Social Care Level 2, Literacy and Numeracy Level 2, and achieving competencies within a maximum of two years. Person Specification Essential – Experience Previous experience as a Band 2/3 HCA or equivalent in a care environment. Desirable – Experience Previous experience delivering physical health interventions such as taking blood pressure, ECGS or venipuncture. Previous experience working with patients with mental health difficulties. Essential – Qualifications Diploma in Health and Social Care Level 2 or equivalent (trainee must achieve within 2 years of commencement). Care Certificate (trainee must achieve within 12 weeks of commencement). Key skills in Literacy and Numeracy Level 2. Completion of the Trust's full HCA Framework, including key skills in Literacy and Numeracy Level 2, within 2 years of commencement. Essential – Skills Effective verbal and written communication skills. Competency in record keeping.
