Main duties of the job Reporting to Trafford Ward Manager, you will assist the team to deliver personal care to our patients who have a variety of rehabilitation and end‑of‑life care needs for Cambridgeshire and Peterborough NHS Foundation Trust. Working with a team of passionate and dedicated health‑care professionals, you will be responsible for delivering a high level of care in our community wards. This post is open to professionals with the right attributes to enhance a team. It involves rotational shifts across 24 hours. Ensure the delivery of high quality, patient‑focused care and treatment which promotes the “Essence of Care” and “Standards for Better Health” principles. Set the Expected Date of Discharge (EDD) through negotiation between patient, carer, team and internal/external agencies to achieve a safe and effective discharge that closely meets the patient's needs. Communicate effectively with patients and their families /carers. Deliver effective patient‑centred rehabilitation care programmes, through appropriate leadership. Monitor quality and compliance with standards continuously within the service. Maintain a safe and healthy environment for patients/clients, visitors and staff. Work across inpatient units if required. Support the nurse in charge in inpatient units in maintaining essential clinical stocks. Promote a positive service user experience through proactive listening, responsiveness to issues raised and resolving concerns with colleagues and senior members of the team. Maintain professionalism towards service users, carers, colleagues and other agencies at all times. Collaborate with other Trust services and external agencies to provide seamless care for service users. Observe and report to a registered member of the team any changes indicating an alteration in the general wellbeing and safety of a service user. Provide service users with personal care, where and when required, demonstrating empathy, dignity, respect and compassion. Ensure nursing notes are current, relevant and accurate, and are subsequently reported & counter‑signed. Partner with the service user to access activities and services that maintain independence and promote healthy lifestyles. Ensure the ward environment is clean and tidy at all times, liaising with the ward housekeeper regarding cleanliness. Support registered staff with the physical health, comfort and wellbeing of service users, including physical observations and basic measurements under guidance. Person Specification Education/Qualifications Registered Nurse NMC Registration Post graduate qualification relating to ward specialism Experience Experience of in‑patient services and working as part of a multi‑disciplinary team Experience of providing mentorship Experience relating to ward specialism Skills & Abilities Commitment to high quality care and service provision Effective communication skills both verbally and written, including IT literacy (MS Office). Team working, role modelling, and empowering other team members. Ability to carry out physical interventions (certain posts only). Use of own initiative to contribute to a multi‑disciplinary team approach. Calmness and appropriate response under pressure. Effective time management and organisational skills. Assertiveness skills. Decision making, delegation and problem‑solving skills. Good understanding of care planning. Knowledge of physical healthcare needs. Commitment to and understanding of evidence‑based practice. Awareness of safeguarding issues and responsibilities under relevant legislation. Basic understanding of the needs of service users. Basic understanding of legislation relating to care including the Deprivation of Liberty Safeguards Act, Children’s Act, Equality Act 2010, Health and Safety Act and the Data Protection Act. Other Recognise peoples’ right to privacy and dignity, treating every person with respect. Willingness to embrace integrated model and new ways of working. Willingness to follow instruction while also questioning and challenging decisions. Commit to professional development and participate in annual appraisal and performance management processes. Interest in user and carer involvement. Physical Requirements Physically able to undertake a demanding role safely, as and when required.
