We are looking to recruit an experienced Band 7 Social Worker to join our established Adolescent Outreach Team (AOT). The team is multidisciplinary, highly committed, and focused on supporting young people to remain at home or, when necessary, facilitating safe discharge from hospital to reduce Length of Stay. Responsibilities Provide care coordination and assessment for an agreed caseload within AOT. Serve as the primary Social Worker, delivering individual, family, group work and telephone support. Liaise and interface with multidisciplinary staff within the service area. Collaborate with external agencies – particularly Local Authority Children’s Services, Tier 3 CAMHS Community Services. Investigate and advise on complex, highly sensitive, and confidential safeguarding cases. Produce written reports for social circumstances, tribunals, managerial hearings, safeguarding investigations, and court communications. Deliver consultative input to clinicians and multi‑disciplinary teams across health, education, housing, police, CJS, voluntary, and statutory agencies. Supervise less experienced social workers in day‑to‑day clinical practice. Maintain up‑to‑date knowledge of relevant legislation, guidance, and Trust policies. Uphold high standards of professional practice, working in the best interests of service users and carers. Collaborate closely with all relevant service members to achieve optimal outcomes. Lead on adult and child safeguarding within the service area and coordinate with local and regional statutory safeguarding and child‑protection services.
