OverviewA NUMBER OF INDIVIDUAL PATIENT ACTIVITY ADVISORS (IPA) ROLES AVAILABLE FOR INDIVIDUAL PATIENT ACTIVITY ADULT TEAMS THROUGHOUT LANCASHIRE & SOUTH CUMBRIA PLACE BASES INCLUDE: BLACKPOOL, FYLDE & WYRE, MORECAMBE, SOUTH CUMBRIA, LANCASTER, BLACKBURN WITH DARWEN, GREATER PRESTON, CHORLEY & SOUTH RIBBLE, EAST LANCASHIRE & WEST LANCASHIRE ROLE IS FAMILY FRIENDLY AND OPEN TO ALL ALLIED HEALTH PROFESSIONALS & NURSES We are looking for candidates across the Lancashire and South Cumbria area. The post holder will have a lead role working within the Individual Patient Activity (IPA) Adult Team concerning the commissioning and reviewing arrangements associated with our adult complex client placements. This includes S117 aftercare, Non-CHC funded cases & Joint funded packages of care with Local Authority. The post holder will take a lead role in attending case reviews including, Care Program Approach (CPA), monitoring standards of care and treatment and liaising with statutory and non-statutory agencies in the repatriation of Lancashire & South Cumbria residents. The post holder will attend Triage and PHB meetings to assist MDT members with funding decisions. The post is a mix of agile working with some office based working, as well as visiting people within their own homes & in placements. Currently we are looking for a motivated and experienced RMN, RGN, RNLD or AHP who can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review process within our IPA Adult Team.Primary ResponsibilitiesMonitor progress on treatments to:Inform future commissioning arrangementsAssist in identifying future resource implicationsEnsure informed information is available on care and treatment as required to assist the commissioningInvestigate sources and reasons for referrals and to influence future referral patternsDetermine compliance with the ICB commissioning policiesSupport the complex MDT meetings in ensuring the most effective care/treatment of patients in the most appropriate locationAchieve the most effective use of the ICB's resourcesMonitor compliance with terms of authorised referralsInitial visits to potential providers and ongoing attendance at relevant review meetingsLiaison with client's key workers from statutory and non-statutory agenciesProvide and receive highly complex, sensitive information. Communicate complex client condition related information to health and Social Care professionals/clients and other groups of staffTo keep up to date with Department of Health Guidance Circulars and ensure proper implementation of policies and proceduresLancashire & South Cumbria ICB is a fully-authorised ICB responsible for the full range of ICB statutory duties and powers. The role is varied and involves working closely with Local Authority, Acute Trusts and with providers You will work within the Mental Capacity Act and gain experience involving Best Interest decisions and Court of Protection work. In the upcoming weeks and months, the organisation plans to initiate a programme of organisational change, though the specifics are yet to be determined. For the individual patient activity (IPA) team, there will be no immediate changes except possibly in line management and portfolio alignment.
