To work as part of a multidisciplinary team within the GP surgery,providing expert clinical pharmacy services to improve patient care andoptimise medicines use. The post-holder will be an Independent Prescriber andwill have completed the CPPE Clinical Pharmacist Pathway as a minimum. Theywill contribute to safe, effective, and evidence-based prescribing, supportingchronic disease management, medicines optimisation, and reducing workload forGPs. Main duties of the job Clinical Care & Prescribing Manage patients with long-term conditions through medication reviews and prescribing within scope of competence. Provide clinical input for acute and repeat prescriptions, ensuring safety and cost-effectiveness. Support medication reconciliation post-discharge and during care transitions Risk Stratification Identification of cohorts of patients at high risk of harm from medicines through practice computer searches. This might include risks that are patient related, medicine related, or both MedicinesOptimisation Conduct structured medication reviews for polypharmacy and high-risk patients. Implement national and local prescribing guidelines and quality improvement initiatives. Identify and manage medicines-related risks and contribute to reducing avoidable harm Collaborative Working Work closely with GPs, nurses, and other healthcare professionals to deliver integrated care. Participate in multidisciplinary meetings and contribute to clinical decision-making Patient Education & Support Provide advice on medicines use, adherence, and lifestyle interventions. Support self-management and shared decision-making with patients Professional Development Maintain up-to-date clinical knowledge and skills through CPD. Person Specification Other requirements Professional, calm and efficient manner Positive and flexible approach to work Hold a Valid UK Driving License Basic life support training Qualifications Completion of CPPE Clinical Pharmacist Pathway training. Independent prescriber Experience in primary care or medicines optimisation. Experience Minimum 2 years post qualification experience, demonstrated within a practice portfolio Previous experience of providing prescribing advice or support General Practice Experience of delivering patient clinics or services Registration Mandatory registration with General Pharmaceutical Council GPhC Skills & Knowledge In depth therapeutic and clinical knowledge and understanding of the principles of evidence‑based healthcare Excellent interpersonal, influencing and negotiating skills Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients) Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions Computer literate with an ability to use the required GP clinical systems (desirable) and Microsoft office packages (essential) Able to obtain and analyse complex technical information and familiarity with EMIS and NHS prescribing systems. Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate Able to work under pressure and to meet deadlines Produce timely and informative reports Ability to work autonomously and as part of a team.
