One of our Partners is retiring and we are seeking a Salaried GP, with a view to Partnership.We are offering 2 - 8 sessions per week with the opportunity to job share. We have a great team of staff including 3 GP Partners, a Salaried GP, Outreach Nurse, Nurse Practitioner, 3 Practice Nurses, 3 HCAs, a GPA, Social Prescriber and Pharmacy Technician.We are a dispensing practice with 7000 patients for whom we dispense for one third.Main duties of the jobThe role would include face to face, telephone or online consultations and home visits when necessary.About usWe are located in the pretty market town of Diss which is approximately 25 miles south of Norwich.Our team of staff is friendly and supportive to all colleagues. We have regular meetings, each Monday lunchtime for an hour where we discuss subjects including prescribing, deaths, clinical cases, etc and lunch is provided.We care about our staff and colleagues and do our best to support them all.Job responsibilitiesThe Lawns Medical Practice - DissJOB DESCRIPTIONJOB TITLE:SALARIED GENERAL PRACTITIONERREPORTS TO:GP Partners (Clinically)Managing Partner (Administratively)HOURS:TBAJob Summary:The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.Clinical Responsibilities:In accordance with the Practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashionMaking professional, autonomous decisions in relation to presenting problems, whether self‑referred or referred from other health care workers within the organisationAssessing the health care needs of patients with undifferentiated and undiagnosed problemsScreening patients for disease risk factors and early signs of illnessIn consultation with patients and in line with current Practice disease management protocols, developing care plans for healthProviding counselling and health educationAdmitting or discharging patients to and from the caseload and referring to other care providers as appropriateRecording clear and contemporaneous consultation notes to agreed standardsCompiling and issuing computer‑generated acute and repeat prescriptions (avoiding hand‑written prescriptions whenever possible)Prescribing in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriateIn general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.Other Responsibilities within the Organisation:Awareness of and compliance with all relevant Practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safetyA commitment to life‑long learning and audit to ensure evidence‑based best practiceContributing to evaluation/audit and clinical standard setting within the organisationContributing to the development of computer‑based patient recordsContributing to the summarising of patient records and Read‑Coding patient dataAttending training and events organised by the Practice or other agencies, where appropriate.Confidentiality:In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters.
