Job Summary This post is only open to current employees of the Herefordshire & Worcestershire Health and Care NHS Trust, Worcestershire Acute Hospitals NHS Trust & ICB. This post is for 22.5 hours per week, to work over 3 days – between Monday and Friday. The post will align with the clinical tissue viability nursing service. The team is a countywide team that also supports services in Herefordshire, offering advice and clinical expertise in wound care across all services within the organisation, nursing homes, and practice nurses. This role requires the holder to travel across the county to meet with teams and support them from a tissue viability quality governance perspective, including neighbourhood teams, community hospitals and specialist teams. Occasional work from home may be appropriate. The post offers a unique opportunity to impact the quality of tissue viability care for the Trust. Main Duties The role supports the delivery of excellent tissue viability care across Herefordshire and Worcestershire Health and Care Trust and in the community by facilitating governance processes and ensuring compliance with clinical standards. Responsibilities include managing risks, promoting continuous improvement in wound care and pressure ulcer prevention, keeping policies up to date and evidence‑based, undertaking audits and quality improvement initiatives, and delivering education across all clinical teams. Key tasks: Visit community and ward‑based clinical teams to identify gaps and provide bespoke training packages. Work closely with the Director of Nursing, Tissue Viability Nursing Lead and the tissue viability nursing team to support process improvement, KPI achievement and embedding the core capabilities framework. Actively participate in the patient safety incident response process across the organisation for tissue viability, responsible for action planning and supporting teams to deliver required actions in line with continuous improvement processes. BSc (Hons) or equivalent qualification and/or experience. Educated to a Master’s level or equivalent qualification and/or experience. A teaching and assessing or mentorship qualification or relevant equivalent experience. Specialist qualification in Tissue Viability or equivalent experience. Understand that original certificates will be required to verify qualifications at offer. Person Specification – Experience Significant post‑registration nursing experience with a strong background in wound care and tissue viability. Experience of mentorship. Experience in developing, reviewing and implementing guidelines/protocols and care pathways. Proven record of delivering education and training programmes to multidisciplinary teams. Demonstrable evidence of continued professional development. Experience in leading clinical practice and service development. Experience in audit, policy development and clinical governance.
