Overview This post will be offered as Band 6 to 7 Progression Pharmacist or Band 7 Clinical Pharmacist (depending on experience). You will be responsible for providing a clinical pharmacy service to designated mental health inpatient teams and delivering on all aspects of medicines optimisation. You will be the source of expert pharmaceutical advice for the MDT and undertake patient‑centred medicines discussions. You will also need to have the skills to be able to work effectively within the wider multi‑disciplinary team of nurses and doctors and other allied healthcare professionals and managers. You will want to develop and improve the reliability and quality of our clinical pharmacy service and partake in Quality Improvement projects. It is imperative that the successful candidate is able to show compassion and respect for people with mental health illness and be driven to improve their care through the safe and effective use of medicines. The post holder will be expected to work with the Clinical Lead/Lead Pharmacist to develop clinical pharmacy services within the Tower Hamlets directorates. They will be responsible for implementing and monitoring the Trust's clinical pharmacy standards as well as ensuring the clinical service is of high quality and patient‑focused. They will identify gaps and problem areas in the service and will use Quality Improvement methodology to make positive changes. The postholder will be required to work closely with the clinical lead ensuring that stock control of medicines on the wards is accurate and that cost efficient measures are taken to improve medicine use across the inpatient wards. The post holder will also be expected to manage clozapine supply for inpatient and service users in the community. Responsibilities Provide a clinical pharmacy service to mental health inpatient teams and deliver medicines optimisation. Serve as the expert pharmaceutical advisor to the multi‑disciplinary team and undertake patient‑centred medicines discussions. Work effectively with nurses, doctors, allied health professionals and managers within the wider team. Develop and improve the reliability and quality of the clinical pharmacy service and take part in Quality Improvement projects. Demonstrate compassion and respect for people with mental health illness. Collaborate with the Clinical Lead/Lead Pharmacist to develop services within the Tower Hamlets directorates, implementing and monitoring Trust clinical pharmacy standards. Identify service gaps and use quality‑improvement methodology to make positive changes. Ensure accurate stock control of medicines on wards and adopt cost‑effective measures across inpatient. Manage clozapine supply for inpatients and community service users. Qualifications & Experience Essential Vocational Masters Degree in Pharmacy. Pre‑registration training. Member of the General Pharmaceutical Council and/or Royal Pharmaceutical Society. Ability to recommend, substantiate and communicate medicine‑related information to mental health patients, carers and clinical staff. Previous evaluated experience of mentorship and training skills. Experience undertaking clinical audit. Training and education relevant to hospital pharmacy. Desirable Working towards a higher degree / diploma in clinical pharmacy. Experience in clinical pharmacy in a mental health setting. Experience working with older people. Psychiatry qualification (or willingness to work towards this). Previous experience in providing clinical mental health services. Staff management experience. Monitoring and advising on drug expenditure. Writing procedures, guidelines or protocols. Knowledge and Skills Essential Clinical and critical appraisal skills. Ability to identify and prioritise clinical workload. Clear, precise communication with medical, nursing and pharmacy staff. Awareness of national and local priorities. Ability to identify and manage risks. Ability to evaluate own work and enhance quality of patient care. Awareness of the clinical governance agenda. Basic IT skills, teaching and presentation skills. Ability to meet set targets and organise own time efficiently. Self‑motivated and willing to deputise for senior staff within limits. Takes responsibility for own actions. Desirable Ability to influence junior medical staff. Understanding of local Trust priorities.
