Are you a newly qualified GP (within the last two years,without a substantive post) Pendle East PCN are seeking an enthusiastic,forward-thinking GPs to join our team. Flexible rate, depending on experience.Althoughthe post is 12 months fixed term the PCN will review this when further clarityis available from NHS England on funding.4 sessions plus 1CPD per week with scope to increase at a laterdate.You will benefit from 6 weeks annual leave and 1 week studyleave pro-rata. In practice educational sessions and PLTs are included withinthe 1-week study leave.Main duties of the jobKey Responsibilities:- Provide high-quality, patient-centred care within the aligned practices.- Work collaboratively with a multidisciplinary team,including, INT, nurses, pharmacists, mental health practitioners, first contact physiotherapists and social prescribers, to ensure integrated patient care.- Conduct consultations, assessments, and follow-ups withpatients.- Provide LARC (coils and implants) fitting clinics- Contribute to the management of chronic disease, health promotion, and disease prevention in line with best practices and PCNpriorities.- Participate in clinical audits, training, and qualityimprovement initiatives to enhance patient care within the PCN.- Support the development and implementation of new care pathways that align with ARRS objectives.About usWe are excited tobe recruiting a ARRS GP to be part of Pendle East Primary Care Network (PCN),which is a collaboration of 5 GP Practices within the Lancashire/Yorkshireborder who collectively care for over 47,500 patients.The PCN memberpractices are located at 4 sites: 2 GP Practices within a large modernpurpose-built building and 3 GP Practices a short drive away.The PCN isdetermined to meet the challenges of modern primary care by workinginnovatively and growing our multi-disciplinary team to provide a holisticapproach to our patient population.The PCN teamalready includes; Social Prescribing Link Workers, Health & WellbeingCoach, Clinical Pharmacists, Pharmacy Technicians, Paramedic, Mental HealthPractitioners, Care Coordinator and GP Assistants.Job responsibilitiesRole SummaryThe post-holder will provide general medical services managing a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.Clinical ResponsibilitiesThe post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care. The following outlines the key responsibilities but is not an exhaustive list.In accordance with the practice timetable (Practice timetables will vary dependent upon the practice alignment), as agreed, the post-holder will make them-self available to undertake a variety of duties including surgery consultations, video consultations, telephone consultations and queries, visiting patients at home, ward rounds, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.Assessing the health care needs of patients with undifferentiated and undiagnosed problemsScreening patients for disease risk factors and early signs of illnessIn consultation with patients and in line with current practice disease management protocols, developing care plans for health.Providing advice and health educationAdmitting or discharging patients to and from the caseload and referring to other care providers as appropriateRecording clear and contemporaneous consultation notes to agreed standards.Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible)Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate.Other ResponsibilitiesAwareness of and compliance with all relevant practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health, and safetyA commitment to life-long learning and audit to ensure evidence-based best practice.Contributing to evaluation/audit and clinical standard setting within the organisationContributing to the development of computer-based patient recordsContributing to the summarising of patient records and coding patient dataAttending training and events organised by the practice or other agencies, where appropriate.Contributing to teaching where appropriateGeneral DutiesTo continue to work until all clinical tasks have been completed.
