You will be fully supported to gain new skills and experience needed to deliver high quality nursing care to patients in their own home or residential setting. As a community nurse you will develop your wound care, palliative care, and long‑term condition knowledge and skills; supporting you to become an expert in assessment and care‑planning to deliver quality care that is caring and compassionate. Travel requirements: Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. Public transport is not acceptable because it does not allow you to meet service needs. We Are Looking For People Who Have experience of working in the community Are educated to RGN level 2 Are able to demonstrate a commitment to high quality care and service provision Have the ability to work flexibly and as part of a team Have the ability to assess and formulate care plans Are able to teach and support student nurses and healthcare assistants Have enhanced communication and interpersonal skills You Will Be Responsible For Keeping the patient at the centre of care, ensuring patient and carer participation in decision‑making Providing a high standard of care Providing a high standard of nursing intervention within a patient’s own home, with access to specialist and advanced practitioners when required Working as part of a 24/7 service and multidisciplinary team Cambridgeshire & Peterborough NHS Foundation Trust is a health & social care organisation dedicated to providing high quality care with compassion to improve the health & wellbeing of the people we care for, as well as supporting & empowering them to lead a fulfilling life.
