We are looking to appoint an Occupational Therapist to join the Neighbourhood Rehabilitation Service (NRS) which includes Intermediate Care, Long Term Conditions and Crisis pathways. The service works across Barnsley Community and provides rehabilitation to patients in their own home and residential care. It also offers rehabilitation to enable earlier discharge from hospital or to prevent hospital admission. You will work within a multi‑professional team, hold a varied caseload and provide therapy input & high‑quality rehabilitation to people within their own homes and local care homes. You will assess, manage & provide treatment to individuals with complex and/or multiple needs, working closely with the MDT and providing supervision/support to other staff. The team is committed to personal development and provides allocated time for CPD, training & wellbeing. A vehicle to undertake travel is essential and the ability to work flexibly including evenings, weekends & bank holidays. Main Duties Be responsible for the assessment & provision of a specialised programme of rehabilitation, meeting the specific needs & agreed goals of patients to facilitate recovery. Manage a clinical caseload with autonomy. Demonstrate knowledge, experience and relevant clinical skills. Monitor and evaluate treatment to measure progress/outcomes. Develop care programmes to support discharge. Contribute to clinical leadership/supervision, facilitating a culture of learning and reflective practice. Communicate effectively with patients, relatives, carers and multidisciplinary team. Ensure compliance with clinical governance and Infection Control policy/procedures. Job Responsibilities Provide a specialist OT service for patients within the Integrated Neighbourhood Team. Supervise, train and develop Basic Grade OT, other health professionals, support staff and OT students. Undertake evidence‑based assessment and treatment of patients, prioritising those with complex needs. Plan and carry out goal‑oriented intervention programmes. Provide OT advice to other professions and assist in administration. Prioritise and manage a significant caseload. Assess for physical, cognitive and perceptual deficits and plan treatment accordingly. Assess, prescribe and evaluate specialised equipment. Communicate complex information clearly to patients, families and agencies. Participate in the induction and training of staff across multi‑professional teams. Provide formal and informal supervision of junior staff and rehabilitation assistants. Teach and support OT students during clinical placement. Maintain own CPD, undertake relevant training and support evidence‑based practice. Maintain accurate and comprehensive patient treatment records. Participate in service development and clinical governance activities. Communication With Others Patients, relatives and carers. Teamwork, independent initiative and awareness of NHS/clinical governance. Risk assessment for own and others’ work areas and caseloads.
