Job DescriptionWe are looking for an experienced Occupational Therapist to join our Transforming Lives Team – Prisons. The role involves assessments, interventions, and care coordination for adult prisoners and community returns, working closely with prison staff, external agencies and other social care professionals.Key ResponsibilitiesConduct occupational therapy assessments and interventions for adult prisoners, ensuring care meets eligibility criteria under the Care Act 2014.Facilitate the transfer of care and support back into the community when a prisoner’s sentence ends.Perform assessments in hospitals, community settings, and relevant environments; prescribe equipment and conduct environmental assessments of housing.Carry out functional assessments, identify appropriate occupational therapy outcomes, commission suitable services and equipment, and develop goal‑setting plans to maximise independence and quality of life.Deliver updates to the wider Social Care team, including Community Forensic Social Work and the Mental Health Discharge Team.As An Ideal Candidate, You Will BeA qualified Occupational Therapist with current registration to the Health and Care Professionals Council.Experienced in working creatively with adults who have care and support needs.Able to communicate, negotiate, organise, and use IT effectively.Committed to inter‑professional collaboration across partner agencies.Autonomous, resilient, and able to work independently in a challenging environment.Holds a UK Driving Licence (unless physically disabled).Appointment is subject to a Disclosure and Barring Service check.
