We are recruiting for a Specialist Respiratory Practitioner within the Integrated Cardio-Respiratory Services. This is a combined service working across hospital and community pathways, providing community services in clinic and home-based settings.The service is a consultant led multidisciplinary team providing a range of service including; diagnostic clinics, COPD and asthma disease optimisation, exacerbation management, hospital discharge, home oxygen, pulmonary rehabilitation and dyspnoea management.The aim is to provide a more accessible, efficient, cost-effective service integrating respiratory services across secondary and primary care, to enable continuity of care and give respiratory patients the opportunities to have access to care closer to their homes and end of life care in their homes.The successful post holder will work closely with the multi-disciplinary team and dedicated social prescriber to improve access to pulmonary rehabilitation and alternative models of therapy for patients who cannot attend pulmonary rehabilitation.They will lead on home rehabilitation and community engagement initiatives with a dedicated focus on improving accessibility within the hard to reach populations.They will have the opportunity to develop quality improvement and project management skills, working with a team of expert clinicians to promote the service through poster presentations, abstract submission and conference attendance.Main duties of the jobTo facilitate and manage the provision of high quality physiotherapy/nursing care to all adult respiratory patients alongside the consultants, AHP's and nurses in respiratory medicine in a variety of settings to promote respiratory health within Imperial College Healthcare and the surrounding community.The respiratory practitioner will provide support and advice to doctors, nursing staff, allied health care specialists, patients and their families/carers, known to the community service, and ensure smooth transitions of care into and within the community setting.The respiratory practitioner will have the opportunity to rotate through different specialisms and settings within the Integrated respiratory service.About usAt Imperial College Healthcare you can achieve extraordinary things with extraordinary people, working with leading clinicians pushing boundaries in patient care.Become part of a vibrant team living our values - expert, kind, collaborative and aspirational. For both overviews please view the Job Description attachment with the job advert.Person SpecificationEducation / QualificationsDegree (or equivalent) in Physiotherapy/NursingState registration with the Health & Care Professions Council/NMC.Evidence of CPD through maintenance of a portfolio.Post graduate study in Respiratory CareExperienceSignificant post registration experience in specialty area and experience in either junior specialty role or team leader management roleExperience with managing patient relevant to specialist areaExperience of working in Pulmonary rehabilitationExperience in integrated respiratory careExperience in oxygen services and acute management of the unwell respiratory patient in the community setting.Skills/Knowledge/ AbilitiesAble to demonstrate highly developed specialist knowledge of own patient groupKnowledge of research and audit including implementation of findings and review of outcomesGood IT skills, Understanding of office Works suite including PowerPointDemonstrate the ability to compile complex written informationDemonstrate commitment through involvement in Trust wide initiatives/ committees.Advanced IT skills use or ExcelDisclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.£56,276 to £63,176 a yearInclusive pro rata #J-18808-Ljbffr
