Then this is the opportunity for you to join us and be part of the C&K CAMHS Crisis Service and Assertive Outreach Team.We are recruiting a Band 5 Mental Health Practitioner into the Assertive Outreach part of our service in Calderdale and Kirklees.The successful applicant will have experience of working in mental health, be able to work autonomously whilst knowing the limitations of the role and will have exemplary record-keeping.This dynamic position will be exactly what you are looking for if you would thrive in a community role where you will be part of a friendly, experienced and supportive team.We welcome newly qualified Nurses/social workers and OTs to apply for this role which will be focused on social integration work and recovery-based therapy principles, care planning and risk assessment.This role is outreach work in the community and so a driving licence will be essential.In addition to all SWYFT/NHS benefits, we offer a supportive induction, supervision, development/training and flexible working within the service where the leadership team are approachable, robust and experienced.All employees of the Trust are strongly encouraged to be fully vaccinated against COVID-19 to protect patients.Main duties of the jobThe post holder will work shifts between the hours of 9am and 5pm, Monday to Friday, under the supervision of a CAMHS AOT Clinicians and will have Clinical Supervision provided by an experienced clinician.The primary role is supporting children, young people and their families, who are open to CAMHS and require intensive support in the community to reach their full potential.The role also involves liaising with other professionals, handing over, assessing young people including care planning and risk assessment.Using Goal Based Outcomes to monitor young peoples journey through CAMHS within a discharge-focused, collaborative approach, is vital to this role.The post holder will use therapeutic engagement skills to inspire young people to maintain their life skills, education and physical activity to aid recovery to ensure children and young people continue to be integrated in the community.Forming and maintaining professional relationships is a key part of the role.We welcome applicants who are: Resilient, work safely under pressure, can prioritise clinical tasks and who can engage and motivate young people to participate in activities.Knowledge of the following legislation is required: Mental Health Act, Mental Capacity.At the time of advertising, this role does meet the minimum requirements set by UK Visas and Immigration to sponsor candidates to work in the UK. Works within a multi-disciplinary team as a member of Specialist CAMHS.KEY RESULT AREAS:To work as a member of an integrated multi-disciplinary team providing comprehensive assessment of children, young people and their carers, planning, implementing and evaluating skilled interventions to address identified needs.To have knowledge of current issues and research relevant to the legislative framework and systems in which children\'s mental health care is delivered e.g. Children Act, Mental Health Act and Care Programme Approach.To contribute to service development within the clinical governance structure and policies and procedures of the Trust. Adhering to NHS guidance outlined by the relevant statutory registration bodies.Promote the safeguarding and welfare of children and young people.ProfessionalAssesses needs and identifies problems relevant to the care of children and young people referred. Devises a plan of care in partnership with the person and carers. Implements the planned programme of care to ensure a high standard is achieved. Reviews the effectiveness of the care provided and where appropriate initiates any action.To be competent in all aspects of care and risk management as defined within appropriate frameworks such as the care programme approach.To work collaboratively with clients, carers and multiple other agencies to provide effective care to optimise and maintain the holistic needs of children and young people with a range of emotional and mental health needs.Management of enquiries and direct referrals of children and young people in accordance with the service Operational Policy and participates in the in hours Single point of Access rota.Maintain professional registration and demonstrate commitment to statutory codes of practice and continued professional development requirements.To be responsible for the safe custody, transportation and administration of any medicines including I.M. preparations.(if appropriate to qualification and / or role )EducationProvide peer supervision and support to other team members and to take personal responsibility and supervise junior staff in the maintenance of written and electronic client and Trust records including inputting data related to own work activity.
