Emotional Well-being Nurse Specialist for the PHN 0-19 serviceThe Croydon School Nursing and Health visiting service is in the process of transforming to a Public Health Service (PHN) 0-19 service. As part of this transformation, a new enhanced pathway has been developed with the main focus on the school-aged Children and Young people to support the emotional wellbeing of the children and young people who attend a Croydon school and who live in Croydon.The post holder will be part of the Croydon 0-19 Public Health Nursing Service, taking the main lead on the enhanced pathway for training and delivery with the outcome of improving the emotional health and wellbeing of Children and Young people on this pathway.Provide early screening and early intervention work in conjunction with other colleagues to children and young people following notified admission to accident and emergency or paediatric wards, following an episode of acute mental health care with a specific criterion.To support role development, the post holder will be expected to hold individual complex cases of children needing direct support and monitoring including safeguarding concerns.The postholder will be accountable for emotional health promotional activities; developing resilience and reducing risks and some direct work with some pupils, supported by the multi-disciplinary team.Interviews: tbcMain duties of the jobTo be the named Emotional Well-being Specialist nurse in Croydon across the three localities within the PHN 0-19 Service. To collaborate with key staff and partners working in the Borough of Croydon and the Croydon Public Health Nursing Service for partnership working.To develop the capability of the Public Health Staff to provide initial assessment and early detection of mental health problems and refer to appropriate services as required.To monitor and provide support and advice to an identified caseload of children and young people. These children and young people may have identified health needs and /or developmental or behavioural difficulties; issues giving rise to safeguarding concerns, and/or may be children looked after by the local authority.To act as the lead professional ensuring entry into early intervention, or referral onto specialist servicesFacilitate health-enhancing activities and lead on areas of practice, for example, specific health issues, clinical governance and standards as directed by the PHN operational Lead.To provide child specific or whole service training sessions on issues related to managing emotional wellbeing healthAbout usCroydon Health Services NHS Trust provide hospital and community services from a number of community and specialist clinics throughout Croydon.
