Primary Care Network Senior Clinical Pharmacist Join to apply for the Primary Care Network Senior Clinical Pharmacist role at Greenwich Health Limited Job Summary The post holder will be employed by Greenwich Health the GP Federation and work with Riverview Health PCN, a grouping of five practices. The role is patient‑facing and involves delivering practice support as well as PCN‑level work, working within a multidisciplinary team to proactively manage patients with co‑morbities, support general practice staff with prescription and medication queries, and provide clinical leadership on medicines optimisation and quality improvement. The holder will manage aspects of the quality and outcomes framework, enhanced services and medicine safety at the practice and PCN levels. Responsibilities Hold patient facing reviews and provide medicines support. Manage medicines in discharges from hospital. Hold long‑term conditions clinics. Provide patient telephone support. Be a centre of medicines information for practice staff. Support and adhere to repeat prescribing processes. Support medicines safety and local/national guidelines. Provide primary support for prescription and medication queries, including repeat prescriptions, acute requests, medicines reconciliation on transfer of care and safe prescribing systems. Provide clinical leadership on medicines optimisation and quality improvement. Manage aspects of the quality and outcomes framework and enhanced services. Qualifications Essential Master’s degree in Pharmacy. Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience. Minimum 5 years post‑qualification experience, with in‑depth therapeutic and clinical knowledge. Registration with the General Pharmaceutical Council. Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences. Excellent interpersonal, influencing and negotiating skills. Demonstrates accountability for delivering professional expertise and direct service provision. Desirable Independent Prescriber. Membership of the Royal Pharmaceutical Society or working towards Faculty membership. Ability to make prescribing changes (as an independent prescriber). Experience in medicines optimisation and quality improvement projects. Person Specification Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences. Contract and Compensation Salary: £48,840 to £56,610 per year. Contract: Permanent. Working pattern: Flexible working.
