Oxfordshire Stroke Rehabilitation Unit at Abingdon Community Hospital is delighted to offer the opportunity for a full time Band 6 Occupational Therapist (OT) within our service. We're a supportive, well-established, and knowledgeable community hospital multi-disciplinary team. You will receive quality individual, safeguarding and reflective practice supervision. We are a progressive service keen to look forward and find ways to develop and improve the service. Main duties of the job To work as part of the specialist stroke rehabilitation team to provide a high-quality rehabilitation service, in line with current best evidence-based practice, for stroke patients on the Oxfordshire stroke pathway. To empower clients to manage their conditions effectively and maximise their potential to reach their goals of self-management, access to their environment and return to previous occupations and involvement in the community. This role would suit someone who is experienced in conducting specific physical and cognitive/functional assessments and delivering interventions to support stroke rehabilitation. The post holder should be able to communicate this effectively within the MDT and family but also externally to key organisations such as social care and specialist services. You will also have responsibility for prescribing equipment and assessing home environments as appropriate. You will also be expected to represent and lead the service at family meetings, and be aware of the protocols and procedures in place to ensure clinical governance and adherence to standards. Undertake specialist neurological assessments of complex stroke patients as an autonomous practitioner, using evidence-based practice and advanced clinical reasoning to guide treatment. Develop and implement patient-specific programmes of stroke rehabilitation, setting measurable goals, evaluating progress, and adapting treatment to optimise recovery. Work collaboratively with the stroke MDT to provide high-quality neurological rehabilitation, ensuring adherence to national stroke guidelines and effective interagency liaison for seamless patient care. Person Specification Knowledge Previous NHS or Adult Social Care experience. Previous experience as Band 5 or equivalent. Experience of working within a multidisciplinary team. Good working knowledge of rehabilitation. Good knowledge of community services. Qualifications Professional Qualification (degree or equivalent). Professional Registration with HCPC. Evidence of continuing professional development (CPD) portfolio. Post graduate education/training specifically relating to patient group. Post registration training in Stroke/rehabilition. Member of RCOT. Member of specialist interest group e.g SSNAP. Experience Good understanding of clinical governance. Previous experience as a Band 5 or equivalent in neurological rehabilitation. Experience working within an interdisciplinary team. Experience in supervision of student placements. Experience with routine and complex hospital discharge planning. Experience/training in cognitive assessment and rehabilitation. Experience participating in family meetings/case conferences. Contractual Hold current driving licence. Able to work flexibly as post may require weekend working. Understanding of equality and diversity. Car owner. Further Training Practice educators course or equivalent. Experience initiating and leading service improvement projects and audits.
