Bolton Living Well Service is recruiting a Registered Mental Health Nurse, Registered Social Worker or Occupational Therapist for a fixed-term or secondment position of up to 12 months. The role is a key worker within a multidisciplinary team, providing assessment, formulation and triage, and supporting fast access to mental health services. Key Responsibilities Act as a key worker, providing ongoing support to service users. Conduct assessments and formulate care plans as part of the team. Triage and signpost referrals to appropriate services and agencies. Coordinate with community services and third‑sector organisations in Bolton. Participate in duty rosters, including evenings up to 7 pm and occasional weekends, in the Referral and Assessment Hub at Royal Bolton Hospital. Qualifications & Experience Registered in nursing, social work or occupational therapy (or equivalent). Experience in community-based mental health, assessment or triage roles. Good knowledge of local agencies and third‑sector services in Bolton. Ability to liaise with professionals and manage multi‑agency collaboration. Working hours Monday to Friday, 9 am–5 pm. Staff will be called on rota for duty shifts at the Royal Bolton Hospital Referrals and Assessment Hub, typically by 7 pm on duty days. Occasional weekend work may be required. Location Based in either Farnworth or Horwich hubs, depending on clinical need.
