Diabetes Health Adviser Join the Northern Care Alliance NHS Foundation Trust as a Diabetes Health Adviser with the Salford Integrated Diabetes Team. This patient‑facing role focuses on educating and supporting individuals with Type 2 Diabetes, promoting self‑management, and helping those newly introduced to insulin. The successful applicant will work across various Salford locations and will receive comprehensive training and support. This role does not qualify for sponsorship. Main Duties Support the education team within the Integrated Diabetes Team and facilitate therapeutic needs of patients with Type 2 Diabetes. Manage patients within an agreed scope of practice delegated by the supervising Dietitian, carrying out clinical and non‑clinical tasks based on agreed guidance. Deliver individual and group education programmes in community settings, ensuring competency under a Registered Dietitian. Update educational resources and source new materials as required. Develop pathways for patient self‑referral, collaborate with local community groups, GP practices, and pharmacies.
