The PCN Paramedic works as an autonomous practitioner within the multidisciplinary team to provide same day and planned care for patients in the practice and community.Main duties of the jobThis role is a key part of our on-the-day appointment duty team. Operating as a limited company, we balance robust businessmanagement with healthcare excellence.Job responsibilitiesAssess, diagnose, treat and whereappropriate refer or signpost patients presenting with undifferentiated or undiagnosedconditions, within scope of practice.provide same day and planned homevisits, including urgent assessments and follow up.Support care homes with acute visits,proactive rounds and multidisciplinary meetings.Request, perform and interpretappropriate investigations such as ECGs and blood tests, in line with localguidance.Contribute to anticipatory andpersonalised care planning, including for people with long-term conditions andfrailty.Promote self-care and healthimprovement, including signposting to social prescribing and communityservices.Maintain accurate, timely clinicalrecords in the practice clinical system and contribute to audit, governance andquality improvement activity.Work collaboratively with the wider MDT,attending relevant clinical and PCN meetings.Maintain professional standards in linewith HCPC requirements and local policies, including active engagement insupervision and CPD.Support the delivery of QOF, incentive schemes,QIPP and other quality effectiveness initiativesUndertake any tasks consistent with the level ofthe post and the scope of the role, ensuring that work is delivered in a timelyand effective mannerPerson SpecificationQualificationsFull HCPC registration as a Paramedic with no cautions or restrictions.If recruited band 6, you will be expected to complete the Level 7 FCP development roadmap, with course fees fully funded and structured support available, including study leave and clinical supervision.Completion of Roadmap to First Contact Practitioner (FCP) for paramedics in primary care at level 7ExperienceMinimum four years post-registration experience including completion of a newly qualified paramedic two-year consolidation programme and a further two years in an autonomous role equivalent to band 6.Evidence of strong clinical assessment and decision-making skills in urgent or unscheduled care.Ability to work autonomously and as part of a multidisciplinary team.Confident use of clinical IT systems and electronic patient records.Experience in primary care or community-based healthcare.Evidence of continuing professional development that extends scope of practice.Experience in end-of-life care and advance care planning.Disclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. #J-18808-Ljbffr
