We provide quality care to adults with mental ill health, learning disabilities, dementia and complex needs, through supported living. Key Duties and Responsibilities Undertake daily living tasks as agreed with the service user, their family and the professionals involved with the individual service plan. Always work within the policies and procedures of Connected Living. Work alongside care and risk assessments specific to each service user's needs. Complete Health and Safety checks within the service in line with policy and procedures. Be confident to lone work in line with lone worker risk assessment. Ensure safe medication is delivered and ordered to the service users' homes. Provide service users with opportunities to express their preference as to the way tasks are carried out. Maintain and promote choice and independence for the service users we work with. Manage service users' behaviours in the least restrictive way possible. Ensure all daily notes, handovers and any relevant documentation are completed by the end of shift, and high-quality efficient handovers are provided to the oncoming shift. Work as part of a team and wider MDT to support the client's emotional and physical wellbeing. Develop and maintain professional working relationships with service users and the relevant multidisciplinary team to ensure positive outcomes for the service user.
