The Partnership aims to deliver world-class hospital and community services, supported by research and education with Hull York Medical School.The post holder will take responsibility under the direction of the Consultant body to deliver an expert nursing service to patients with Type 1 and Type 2 Diabetes. Some caseloads may include MODY and Cystic Fibrosis related diabetes, from diagnosis until discharge or transition to the adult service. The post holder will work flexibly across hospital and community settings and will provide nurse-led consultations with the child and family.You will educate, inform and support patients and families, providing clinical and social support related to their specific diagnosis, and will provide expert advice to other health care professionals and initiates guideline-based therapies to address acute management issues.
