This is an exciting opportunity to play a vital role in supporting patients and carers to achieve their rehabilitation goals, enhance independence, and improve quality of life.As part of the Multi-Disciplinary Team (MDT), you will work under the guidance of allied health professional (AHP) colleagues to deliver rehabilitation care plans in patients' own homes. You will help patients regain independence and maximise their rehabilitation potentials by implementing therapy programmes, prescribing appropriate equipment, and monitoring progress to ensure care is personalised and effective.CR-STAT team supports Greenwich residents with rehabilitation needs, helping them remain safely at home, maximising quality of life , reducing reliance on care packages, and preventing unnecessary long-term care placements.This is a fantastic opportunity to be part of a supportive and patient-focused team, making a real difference to the lives of residents across Greenwich.Main duties of the jobMaintain own client caseload, as designated by qualified staffUndertake client assessment using structured assessment FormsUndertake a telephone assessment to ascertain the needs of the client and signpost/prioritise as appropriate with the guidance of qualified members of staffDeliver intervention with minimal or no immediate supervision from AHP colleagues often working as a lone practitioner.Recommend appropriate interventions to promote independence and safety following local protocolsMonitor client status, using own judgement and knowledgeProgress and/or make minor modifications to treatment programmes within agreed clinical protocols and parameters, reporting any significant changes to the Health and Social care MDTImplement treatment within care plansUndertake follow up visits to monitor discharge outcomes and encourage continuation of rehabilitation programUndertake multidisciplinary social/ rehabilitation programs with patients/ clients as directed by AHP colleagues, reporting back on progress and outcomes, working with the MDT to ensure programs are current and appropriateDevelop dexterity, co-ordination, palpatory and other therapeutic skills necessary for effective rehabilitation of clients.Maintain effective multidisciplinary communications in order to ensure that treatment goals are met, and a high-quality service is provided to clientsAbout usOxleas offers a wide range of NHS healthcare services to people in community and secure environment settings.
